Short-Term Skilled Nursing and Rehabilitation

High-Quality Therapy and Support That Gets Your Life Back on Course

St. Teresa Rehabilitation & Nursing Center provides a wide range of short-term skilled nursing and rehabilitation services for individuals seeking a safe, quick and effective transition from a hospital stay back home.

Our program includes comprehensive physical, occupational and speech-language therapies designed to fit your needs, reduce your risk for setbacks and restore your ability to perform at the highest degree of independence as possible.

Individualized Rehabilitation Services to Fit Your Unique Needs

Planning your short-term rehabilitation stay at St. Teresa begins before you even arrive.  Our staff makes it as easy on you as possible by working closely with case managers and providers throughout the transition, from hospital discharge to admittance at St. Teresa.

Once you get settled in, our highly-trained team of nurses, rehabilitation therapists, dieticians, social workers and nursing assistants will sit down with you to gain an understanding of your goals and expectations. From there, we’ll develop a personalized plan of care carefully customized to best support you and help you not only reach your goals – but exceed them.

We’ll continue to cheer you on with every win and motivate you to move forward in any challenging moments. Together, we share in the focus and commitment to restoring your full functional ability and getting you back home as soon as possible.

The St. Teresa Approach

Discharging from a hospital to a rehabilitation setting can be a difficult process – you may be nervous, worried, anxious and have many questions. It’s our promise at St. Teresa to reduce the stress on patients and families by making the transition as smooth, comfortable and hassle-free as possible.

Our staff will:

  • Work with case managers, providers and family members to streamline admission.
  • Clearly explain the process from admission to discharge.
  • Help you understand your payment options, including Medicare and commercial health insurance coverage.
  • Put together your own rehab team that works in coordination with your surgeon or primary care physician.
  • Give you the highest level of privacy possible.
  • Be available when you need us to answer questions and address concerns.
  • General or cardiac surgery
  • Joint replacement and orthopedic surgery
  • Cardiac events and interventions (such as stent insertions or treatment for congestive heart failure)
  • Strokes
  • Respiratory Issues

We offer complex nursing care for a wide range of chronic and acute medical conditions.

St. Teresa’s rehabilitation services are provided by licensed Physical, Occupational and Speech-Language Therapists. In addition, our team includes Certified Occupational Therapy Assistants (COTAs) and Physical Therapy Assistants (PTAs).

Having a complete therapy team allows patients to benefit from an integrated approach to therapy. Consistent, repetitive therapy is important for an effective and timely recovery. Rehabilitation services in a skilled nursing facility provide the opportunity to receive therapy more than once a day and at times that are optimal for you. St. Teresa offers that invaluable flexibility, in addition to the highest level of services.

Physical Therapy is provided by licensed Physical Therapists (PTs) who evaluate each patient and design a custom activity and exercise plan that builds strength and improves the range of motion for limbs and joints. PTs help reduce pain, restore functional ability and prevent disability.

Occupational Therapy is provided by licensed Occupational Therapists (OTs) who perform evaluations and help customize therapy that improves the patient’s abilities to perform daily tasks, ranging from self-care to recreational or occupational activities. In addition, OTs design treatment plans and assist patients in improving function, visual, cognitive and general coordination skills. The ultimate goal of occupational therapy is to facilitate a patient’s ability to perform with the highest degree of independence possible.

Speech-Language Therapy is provided by licensed Speech-Language Pathologists (SLPs) who address communication, cognition and dysphagia (swallowing) deficits that may be due to injury, illness or chronic decline. SLPs provide therapeutic interventions to improve receptive and expressive communication disorders, cognitive-communication disorders and speech impairments due to various types of medical conditions and disorders. SLPs also establish communication systems for patients with hearing impairments, and for those requiring alternative communication systems. Additionally, SLPs play a primary role in the evaluation and treatment of adults with swallowing disorders.

Our goal for short-term rehabilitation patients here at St. Teresa is simple: we want to get you home safely – and as soon as possible.

Don’t get us wrong – when you come through our door, you become family and part of a vibrant community. And when you leave, we’ll miss you. But we know the challenges you’re going through, how your limitations may be frustrating and how being back home is at the top of your mind. So, we’ll work with you as hard as we can to get you to that point.

While you’re here, you’ll receive 24/7 nursing care and support. Our staff treats you like their own family members, with compassion, understanding and patience. We’ll be your biggest cheerleaders.

We’ll simulate your normal lifestyle, creating an environment that mimics your everyday life and routines as much as possible. Your dedicated team of physical, occupational and speech therapists is continuously focused on your goals and providing services at times that are best for you and your daily schedule. Our bright rehab gym is the perfect place to keep you motivated, engaged and energized.

You’ll also have access to a variety of services and amenities within our quaint community. This includes an abundance of dining options to meet your needs, whether it’s enjoying delicious cuisine in our Bistro Café with restaurant-style dining, or informal dining in our smaller dining room within steps of your room. You may also have food delivered to your room at any time.

You can enjoy our outdoor courtyard for some downtime, fresh air or time with family. Our library offers many books and as much quiet time as needed. A 24/7 coffee and tea bar is also right there.

Your room comes complete with Wi-Fi, phone and cable television, so you can continually stay connected with loved ones, the world around you or simply enjoy your favorite television program. You may bring almost anything to make you feel comfortable, whether it’s your favorite pillow, photos of your family, your clothing, electronics or personal care items. Visitors – whether family or friends – are always welcome.

And when it’s time to go home, we’ll give you your own personalized send-off, with plenty of cheer (like this one). After all, here at St. Teresa, you’re family.

Resident safety and personalized attention, combined with compassionate care, are the foundation of high-quality healthcare at St. Teresa. We are proud recipients of the AHCA National Bronze Award for Commitment to Quality (2019) and US News and World Report – Best Nursing Homes (2017-2018).

If you have Medicare coverage, you may qualify for the Medicare Skilled Nursing benefit. If you have commercial health insurance through an employer, you may also be eligible for skilled nursing services. Coverage should be verified through your insurance provider or the hospital’s discharge planner. If you have any questions regarding payment, please contact our Business Office Manager or Social Worker at 603-668-2373.
